New! Lifestyle Evaluation and Management (in progress)
(The following are separate files with tools that may be useful to evaluate and encourage changes in the lifestyle of participants of a church, small group, or an interested individual. The 6 areas include (besides the screening questionnaire and description of the program): Nutrition, Motion (activity), Connectedness, Substance Use, Recovery (sleep and stress), and Spiritual Health. Some were created for this program and some are tools that were taken from the Webb to supplement the educational process. This program was inspired by the American Academy of Family Physician’s Lifestyle Medicine program. I have modified the screening tool, and added a spiritual component to better accommodate those with a Christian worldview. I hope you find this helpful and encouraging.)
